We will do our best to accommodate your schedule; if the appointment time offered to you is not workable, please ask the receptionist if other times are available.

Will my insurance cover diabetes education visits?

Yes; Medicare, Blue Cross Blue Shield and other health insurance carriers cover diabetes education and medical nutrition therapy. Checking on the coverage for your policy is recommended and we can help you to determine your benefits.

Individual Consults

All initial education visits are provided on a one-to-one basis.  Your initial visit will include a variety of self-management topics specific to your individual needs. Follow-up visits are also available individually. Visits are usually an hour each, but can be adjusted as needed.

Group Education

Group education is available and preferred by some insurance companies, and may be suggested to you after your initial visit. Group education classes are held four times per year in February, May, August and November.

Diabetes Support Group

It provides a place for continued learning, support, and accountability in the presence of your diabetes educators and your peers.  This group is free of charge and is scheduled on the third Wednesday at noon in January, March, April, June, July, September, October and December (The schedule may be subject to change).  Please call the CMC reception at 652-7179 to confirm dates and times

We look forward to working with you!
